Last week I performed a aortic arch chimney procedure, for total aortic arch stentgrafting, but postoperative iatrogenic coarctation was noted in the next day, the pressure gradient between ascending aortic root and main stentgraft over distal ascending aorta was 50 mmHg,.
Initial I though the more ballooning may relief the condition, but the pressure gradient decrease to 30mmHg but graduately increasing to 50 mmHg.
So I decided to put another stentgraft to relief the stentgraft kinking.
Fortunately, the patient get dramatic improvement.
